HAD AN ACCIDENT AT WORK?

KNOW YOUR RIGHTS!

Use an SC2 self-certification form (SC1 for unemployed or self-employed) to provide your boss with details of sick absences of 4+ days in a row. After 7 days your boss can demand a doctor's certificate.

Use the Internet to get an SC2 at HMRC.gov.uk. You can get a full guide to certificates by doctors from the Department for Work and Pensions (DWP).  You are entitled to SSP for up to 28 weeks -  £79.15 a week as at April 09.
